New Applicants for Open Enrollment

 

Open Enrollment Period is May 3-May 28


Parents of students that are new applicants for Open Enrollment for the 2021-2022 school year must submit all of the following items to apply:  

  1. Inter-district Open Enrollment Application 21-22

  2. Parent agreement for Open Enrollment

  3. Parent/legal guardian current state identification

  4. Original birth certificates (no copies or hospital birth records accepted)

  5. Proof of residency (see list in the sidebar)

  6. Court filed stamped documents (custody, divorce decrees, foster-placement, etc.)

  7. Current & updated immunization records

  8. Attendance records (most recent school year)

  9. Discipline records (most recent school year)

  10. Report card (grades K-8; most recent school year)

  11. Transcript (for students in grades 9-11 only)

  12. IEP and ETR (if applicable)

  13. 504 Plan (if applicable)


The application and all documents must be turned in at the same time. Items submitted separately will not be accepted. 


You can send all of your application documents to [email protected] Please put "Open Enrollment Application" in the subject line of your email. 

OR 

Application & list required documents can be obtained and submitted  at the Trotwood-Madison Board of Education Office located at 3594 N. Snyder Road, Trotwood, Ohio 45426.  Application & all documents can be turned into the Open Enrollment Dropbox, Monday-Friday, 8:00am-4:00pm.


Applications WILL NOT be available at the school buildings.  

Failure to submit your application and all required documents on or before May 28, may cause your child to lose the opportunity to attend Trotwood-Madison City Schools as enrollment is not guaranteed or promised.

Current Open Enrollment Students

Parents of students that are currently enrolled as an Open Enrollment student(s) for the 2020-2021 school year must re-apply for the 2021-2022 school year. 

Parents/Legal Guardians must do the following: 

  1. Complete the 2021-2022 Open Enrollment Application

  2. Provide updated proof of residency.  

  3. Read and sign the Parent Open Enrollment Agreement

Open Enrollment application and Parent Open Enrollment Agreement can be downloaded and completed from our website. Feel free to send your completed application, proof of residency, and Parent Open Enrollment Agreement to [email protected] 

Please put "Open Enrollment Application" in the subject line of your email.

Applications can also be obtained and submitted  at our Board of Education Office located at 3594 N. Snyder Road, Trotwood, Ohio 45426.  Attach and submit all documents into the Open Enrollment Dropbox. 


The deadline for submitting applications for current Open Enrollment students was April 30. However, you may still submit your application with the understanding that enrollment is not guaranteed or promised.
