Kindergarten Registration

Kindergarten

To be eligible for kindergarten, a student must be 5 years old on or before September 30th of that school year.  At the time of registration, the custodial parent must provide the following items:  a legible birth certificate, up to date shot records, a current picture ID, proof of residency (DP&L or Vectren bill-30 days or less, home purchase agreement, or non-generic lease agreement, custody papers if applicable).  All questions should be addressed to the Early Learning Center.

Parents interested in scheduling a tour of the Early Learning Center and our Kindergarten Program can contact the building principal at 854-4511.

Preschool

To be eligible for preschool, a child must be 3 or 4 years of age by September 30, 2015 and live in the Trotwood-Madison City Schools District.  At the time of registration, the custodial parent =must provide the following items:  a legible birth certificate, up to date shot records, a current picture ID, proof of residency (DP&L or Vectren bill-30 days or less, home purchase agreement, or non-generic lease agreement, custody papers if applicable).  All questions should be addressed to the Early Learning Center.

Early Learning Center - 9:30 am - 2:00 pm

To save time, you may print and complete the following forms and bring them with you along with the other required items listed below.

Please be sure to bring the required items with you so that we will be able to assist you promptly and enroll your child in our district.

  • DP&L or Vectren Bill (no more than 30 days old)
  • Home purchase agreement/Deed/Mortgage Statement/Tax Bill/Valid lease agreement
  • If none of the above applies, you may register using a lease issued by the HOMEOWNER and by providing TWO LEGAL DOCUMENTS that have been MAILED TO THE HOME WHERE YOU ARE LIVING in addition to the lease.

  • Unemployment Compensation Documents (mailed to the home)
  • Child Support Documents (mailed to the home)
  • Workers Compensation Documents (mailed to the home)
  • Voter Registration (confirmation mailed to the home)
  • Bank Statement (mailed to the home, not a printout)
  • Court Documents (probation,etc. mailed to the home)
  • Current Driver's License of State ID with Current Address
  • Social Security Documents (mailed to the home)
  • Internal Revenue - City, State, or Federal (most recent, mailed to the home)
  • Current Student Loan Documents (mailed to the home)
  • Water Bill (mailed to the home in name of custodial parent)
  • Cable Bill (mailed to the home in name of custodial parent)
  • Payroll Check Stub (with home address)

All children are entitled to an education, regardless of the severity of their disability. In compliance with Federal and State laws, the Trotwood-Madison City School District offers a number of services for students with disabilities, many of which are incorporated into the regular education program.
